CLIENT CONSENT FORM

For the safety of our staff and clients, the below forms need to be filled out before service may be performed.

These forms may be filled out on a computer using Adobe Acrobat Reader, then printed out and brought with the client to their appointment for signature.  Alternatively, the form can be emailed to Lasting Impression to be printed at our office for signature. 

Please note, that these forms MUST be signed before a Lasting Impression staff member to be valid. Thank you in advance for your understanding.
